What you should know about quitting smoking

Medication can make quitting smoking easier, especially when combined with a clear plan and support in everyday life.

Why quitting is so hard

Nicotine is physically and psychologically addictive. It affects the brain's reward system; if this system is deprived, withdrawal symptoms occur. In addition, ingrained habits play a role, such as smoking a cigarette with coffee or in stressful situations.

Typical withdrawal symptoms in the first few weeks include:

  • strong cravings for cigarettes, irritability and restlessness
  • Concentration and sleep disorders
  • depressed mood and increased appetite

These symptoms usually subside after a few weeks. However, the craving for a cigarette can still occur in certain situations long afterward.

This is how the medications help

  • Varenicline (Champix or generic): It acts directly on the nicotine receptors, can reduce cravings for cigarettes and withdrawal symptoms, and weaken the effect of a smoked cigarette.
  • Cytissinicline (Decigatan): It also binds to nicotine receptors and is used as a 25-day course with a fixed, decreasing dosage schedule.
  • Bupropion (Zyban): It affects the neurotransmitters noradrenaline and dopamine and can alleviate withdrawal symptoms.

All three preparations contain no nicotine, and the quit date is set at the beginning of treatment. Accompanying counseling, such as a smoking cessation course, can further improve the chances of quitting smoking permanently.

Possible side effects

Nausea, sleep disturbances, unusual dreams, and headaches are common with varenicline. Bupropion frequently causes insomnia, dry mouth, and headaches, and can increase blood pressure. Seizures occur in approximately 1 in 1.000 patients treated with bupropion. Increased appetite, sleep disturbances, dry mouth, nausea, rapid heartbeat, and increased blood pressure are among the common side effects of cytisiniline.

Mood swings, depression, and suicidal thoughts have been reported with varenicline and bupropion. However, such symptoms can also occur due to nicotine withdrawal itself. Dizziness and fatigue are also possible; be careful when driving until you know how you react.

When you need immediate medical help

Stop taking the medication and seek immediate medical help if you experience unusual restlessness, severe low mood, changes in behavior, or thoughts of self-harm.

Call the emergency number 112 (Switzerland: 144) in case of:

  • a seizure
  • Swelling of the face, lips or tongue, shortness of breath or severe skin reactions
  • Chest pain or sudden paralysis

Inform your family members about your decision to quit smoking and the treatment you are undergoing, so that they too can watch for changes.

Having a plan to quit smoking

Medication can alleviate withdrawal symptoms, but smoking habits remain a challenge. Therefore, it helps to prepare well for quitting smoking and to involve support, such as through smoking cessation counseling, a course, or people in your social circle.

  • Set a firm quit date for smoking and remove cigarettes, lighters and ashtrays beforehand.
  • Consider alternatives for typical smoking situations, such as a short walk or a glass of water.
  • Exercise regularly and pay attention to your diet, as appetite often increases after quitting smoking.

Especially in the first few weeks, it helps to consciously avoid alcohol and typical smoking situations. Reward yourself for milestones, for example, with the money you've saved. When should I start taking it?

Varenicline and bupropion are started before you actually quit smoking to allow the medication to build up its effects. With varenicline, you typically quit smoking 1 to 2 weeks after starting treatment; with bupropion, it's usually in the second week of treatment. With cytisiniline, you quit smoking no later than the 5th day of treatment. You will find the exact procedure in your prescription and the package leaflet.

A combination of these medications should only be undertaken after consulting a doctor. When bupropion is used with nicotine patches, blood pressure may increase and should be monitored regularly. Studies have shown that varenicline combined with nicotine replacement therapy frequently causes side effects such as nausea, headache, or dizziness. With cytisiniline, additional nicotine may intensify side effects. Please indicate in the questionnaire whether you use nicotine replacement products.

It has been reported that alcohol can have a stronger effect when taking varenicline, sometimes leading to unusual or aggressive behavior. With bupropion, alcohol can increase the risk of seizures and psychiatric side effects; even sudden abstinence after regular heavy consumption is risky. Therefore, drink as little alcohol as possible and answer the questionnaire honestly.

Yes, that's possible. The physical changes after quitting smoking can affect the way some medications work, for example, theophylline, warfarin, or insulin. In that case, a dose adjustment may be necessary. Therefore, inform the doctors who prescribe your other medications about your plan to quit smoking.
