Preparations for hereditary hair loss

Finasteride is a prescription medication, while minoxidil 5% solution is available only from pharmacies in Germany; both preparations are intended for use by men only. What you should know about hair loss

Not all hair loss is hereditary. The cause determines which treatment is appropriate.

Causes and typical signs

Losing up to about 100 hairs a day is considered normal. In men, hereditary hair loss (androgenetic alopecia) is the most common form. In this condition, the hair follicles are sensitive to the hormone dihydrotestosterone (DHT): the hair becomes thinner, shorter, and eventually falls out. Typical symptoms include:

  • receding temples on the forehead
  • a thinning area on the top of the head
  • a slow progression over years

Other causes include stress, infections, iron deficiency, thyroid diseases, or medications.

Treatment options

Hereditary hair loss cannot be cured, but its progression can be slowed. Medical options include:

  • Finasteride (Propecia or generic): A tablet that inhibits the formation of DHT. The tablet is taken once daily and is only approved for men.
  • Minoxidil: Solution or foam for application to the scalp. The active ingredient prolongs the growth phase of the hair follicles. The 5% minoxidil solution offered here is applied twice daily and is approved for use by men only in Germany.
  • Hair transplantation: Surgical procedure, usually for advanced hair loss.

Drug treatments are only effective as long as they are continued, and initial results usually only become apparent after several months.

Possible side effects

Finasteride: Occasionally, decreased libido, erectile dysfunction, or reduced semen volume may occur; it has been reported that such symptoms can persist after discontinuation of the medication. Other possible side effects include breast tenderness or enlargement, skin rash, and mood changes, including depression and suicidal thoughts. Finasteride lowers PSA levels in the blood. Therefore, inform your doctor before any prostate examination.

Minoxidil: Common side effects include headaches, itching or flaking of the scalp, and unwanted hair growth, for example on the face. Increased hair loss may occur temporarily during the first few weeks. Dizziness, rapid heartbeat, or swollen hands and feet are also possible.

When you should see a local doctor

Have hair loss examined in person if it occurs suddenly, in patches, or in circular patterns, if the scalp is itchy, flaky, red, or scarred, or if symptoms such as fatigue, weight changes, or fever occur. Hair loss in women and children should also be investigated in person.

Consult a doctor promptly if you experience lumps, pain, or discharge in the chest, or a depressed mood while taking finasteride. This also applies to minoxidil if you experience chest pain, rapid heartbeat, shortness of breath, fainting, dizziness, or swollen hands and feet. Call emergency services 112 (Switzerland: 144) if you experience severe chest pain, suicidal thoughts, or signs of a severe allergic reaction such as swelling of the lips, tongue, or face with difficulty breathing.

Patience is part of the treatment

Hair grows in long cycles, so the effects of treatment only become apparent after months. Realistic expectations help to put changes into perspective. The primary goal is to slow the progression of hair loss. Finasteride is approved for early stages; its effectiveness in advanced stages of hair loss has not been proven.

  • Take photos in the same lighting conditions at the beginning to reliably compare the progress after 6 and 12 months.
  • Ensure you have a balanced diet with sufficient protein and iron, as strict diets can promote hair loss.
  • Treat your hair gently: use minimal heat, avoid tightly bound hairstyles, and employ mild shampoos.

According to the product information, the effects achieved with finasteride are lost again within approximately 9 to 12 months once it is discontinued. Can women also take finasteride?

No. Finasteride is only approved for use in men. In women who are pregnant or could become pregnant, the active ingredient can interfere with the development of the sex organs of a male fetus. These women should therefore not touch broken or crushed tablets. Hair loss in women should be evaluated by a doctor.

Hair grows slowly. With finasteride, daily use typically requires three to six months before it becomes apparent whether hair loss stabilizes. With minoxidil solution, it usually takes two to four months to see if the treatment is effective; more hair may fall out temporarily during the first few weeks. The effect varies from man to man. Your doctor will decide whether to continue treatment based on your individual progress.

The effect only lasts as long as treatment is continued. According to the product information, after discontinuing finasteride, the effects achieved are gradually lost; after about 9 to 12 months, the condition usually returns to its baseline level.
