Preparations for hay fever

The nasal sprays and Grazax require a prescription; cetirizine and loratadine are available only from pharmacies. After making your selection, you will complete a medical questionnaire, and a doctor will decide whether a treatment is suitable for you.

Which treatment might be suitable?

  1. 1 When sneezing, itching and a runny nose are the main symptoms Antihistamines such as cetirizine or loratadine are taken as a tablet once daily and can also be used for itchy eyes or hives. Drowsiness is a possible side effect. A doctor will decide which medication is suitable.
  2. 2 Especially if the nose is blocked or if the symptoms persist for weeks. Cortisone nasal sprays like Avamys, Flixonase, or Nasonex work directly in the nose and are used regularly every day. The full effect develops over several days. A doctor will determine whether a particular spray is suitable.
  3. 3 For moderate to severe symptoms that are not sufficiently relieved by a single nasal spray. The combination nasal sprays Dymista and Ryaltris contain a cortisone and an antihistamine. No additional cortisone-containing nasal spray is used. A doctor will decide whether a combination is appropriate.
  4. 4 If your grass pollen allergy has been tested and immunotherapy has already been started by a doctor. Grazax is taken daily for three years, starting before the pollen season, and is not a remedy for rapid relief. It is not suitable for severe or uncontrolled asthma. The doctor will decide whether treatment can be continued.

What you should know about hay fever

Hay fever is a common allergy in which the immune system overreacts to otherwise harmless substances such as pollen. Medication can alleviate symptoms, but it does not replace the need to investigate new or unclear symptoms.

What is hay fever?

In hay fever (seasonal allergic rhinitis), the immune system reacts to pollen from trees, grasses, or herbs. This reaction releases, among other things, the messenger substance histamine. Typical symptoms include:

  • Sneezing fits, a runny or blocked nose
  • Itching in the nose, eyes, or palate
  • Red, watery eyes and fatigue

If similar symptoms occur throughout the year, an allergy to house dust mites, pet dander, or mold is often the cause (perennial allergic rhinitis). Whether an allergy is present and what triggers it can be determined with a skin or blood test.

What treatments are there?

Depending on the type and severity of the symptoms, various medications may be suitable:

  • Antihistamines: Tablets such as cetirizine or loratadine block the effects of histamine.
  • Cortisone nasal sprays: They inhibit allergic inflammation directly in the nose
  • Combination sprays: combine a cortisone with an antihistamine
  • Immunotherapy (hyposensitization): The immune system becomes accustomed to the allergen over the years.

Antihistamines and nasal sprays alleviate symptoms as long as they are used, but they do not cure the allergy. Immunotherapy with Grazax requires confirmation of a grass pollen allergy and is initiated by a doctor.

Side effects and cautions

Cortisone nasal sprays can cause nosebleeds, dryness, burning in the nose, or headaches. Only a small amount of the active ingredient enters the bloodstream. Blurred vision or other visual disturbances should be evaluated by a doctor. Caution is advised when using them with HIV medications such as ritonavir or cobicistat.

Antihistamines can cause drowsiness, especially when combined with alcohol, and thus impair driving ability. Itching and swelling in the mouth are common side effects of Grazax.

Do not use multiple cortisone-containing nasal sprays at the same time and list all medications in the questionnaire.

When should you see a doctor immediately?

An allergic reaction can rarely be severe. Call emergency services immediately on 112 (Switzerland: 144) if:

  • Swelling of lips, tongue, face or throat
  • Shortness of breath, wheezing, or a feeling of tightness in the throat may occur.
  • Dizziness, rapid heartbeat or fainting may occur together with allergic symptoms.

Seek prompt medical attention if your nose is permanently blocked on only one side, if you experience repeated nosebleeds, crusting or facial pain, if you develop a fever, or if you develop a cough and breathing difficulties.

Pollen in everyday life

Reduce contact with pollen

In addition to medication, reducing contact with the allergen can provide relief. A pollen calendar and regional pollen forecast show when which pollen types are in the air. Cortisone nasal sprays work best when used regularly and starting at the beginning of the pollen season. With immunotherapy using Grazax, timing is even more crucial, as treatment begins approximately four months before the grass pollen season.

  • Wash your hair before going to bed and don't leave street clothes in the bedroom.
  • In the city, ventilate in the morning; in the countryside, in the evening.
  • Keep the windows closed and use a pollen filter while driving.

If symptoms persist despite treatment, or if coughing and shortness of breath develop, an evaluation by an allergist is advisable. Do I need a prescription for these medications?

It depends on the medication. The nasal sprays Avamys, Flixonase, Nasonex, Dymista, and Ryaltris, as well as Grazax, require a prescription. Cetirizine and loratadine are available only from a pharmacy. Even with these tablets, a doctor will review your questionnaire before the partner pharmacy ships the medication. Therefore, please provide complete information about any pre-existing conditions and medications you are currently taking.

Initial improvement is sometimes possible after a few hours, but the full effect usually develops over several days.
