What you should know about testosterone deficiency

A testosterone deficiency can only be reliably diagnosed through typical symptoms and repeated blood tests. Treatment replaces the missing hormone and is not a means of building muscle or improving performance.

What is a testosterone deficiency?

Testosterone is the most important male sex hormone. It is primarily produced in the testicles and regulated by the pituitary gland. If the body consistently produces too little of it, this is called hypogonadism. Possible symptoms include:

  • decreased libido and erection problems
  • persistent fatigue, lack of energy or depressed mood
  • Decrease in muscle strength and body hair

These symptoms are nonspecific and often have other causes. Therefore, testosterone levels are measured at least twice, usually in the morning, and the cause of the deficiency is investigated.

How do testosterone gels work?

Testosterone gels replace the missing hormone. The gel is applied once daily to clean, dry, and healthy skin; the active ingredient is absorbed through the skin into the bloodstream.

  • Testavan: 20 mg/g, with applicator on upper arm and shoulder
  • Testogel: 16,2 mg/g, from a dosing dispenser or bag, applied to shoulders or upper arms
  • Tostran: 2%, on the abdomen or inner thighs

The dose is adjusted based on blood test results; the timing of the blood draw depends on the specific product. Never apply to the penis, testicles, or broken skin. Testosterone is intended only for men with a confirmed deficiency.

Side effects and cautions

Possible side effects include skin irritation at the application site, an increase in red blood cells (hematocrit), elevated blood pressure, and a rising PSA level. Acne, fluid retention, or breast enlargement can also occur. Therefore, testosterone levels, blood count, PSA level, and prostate health are checked before starting treatment and regularly thereafter.

Testosterone should not be used in cases of prostate or breast cancer. Testosterone can temporarily inhibit sperm production and can be transferred to other people through skin contact. Pregnant women, breastfeeding women and children should not touch the treated skin. The gel is flammable until it dries.

  • Swelling of the face, lips or throat, or difficulty breathing
  • a painful erection lasting for several hours

See a local doctor promptly if you experience swollen ankles or rapid weight gain, yellowing of the skin or eyes, sleep apnea, or difficulty urinating. If women or children in your community develop acne, increased body hair, or a deeper voice, have this checked out as well.

Testosterone gel treatment is usually long-term and requires regular blood tests. The first checkups take place a few weeks after starting, followed by longer intervals. These checkups measure testosterone levels, hematocrit, and PSA levels, and adjust the dosage if necessary. It's best to note down the appointment times right from the start. Simple habits can help prevent testosterone from spreading to other people.

  • Immediately wash off any gel on your hands with soap and water and cover the dry area with clothing.
  • Wash the area before close skin contact, at the earliest after the waiting period specified in the package insert.
  • Keep the gel away from flames until dry and out of reach of children.

Significant obesity, lack of sleep, and excessive alcohol consumption can contribute to low testosterone levels; therefore, a healthy lifestyle remains important even during treatment.
