Ulcerative colitis and Crohn's disease vary greatly from person to person. Close medical supervision and regular check-ups are the foundation of any treatment.

What are ulcerative colitis and Crohn's disease?

Both diseases belong to the group of chronic inflammatory bowel diseases (IBD). They often begin in young adulthood and progress in flare-ups with periods of remission in between.

  • Ulcerative colitis: The inflammation affects the lining of the large intestine, begins in the rectum, and spreads to varying degrees. Typical symptoms include bloody, mucus-filled diarrhea and a strong urge to defecate.
  • Crohn's disease: The inflammation can affect any part of the digestive tract, most commonly the end of the small intestine. Typical symptoms include abdominal pain, diarrhea, and weight loss.

Joints, skin, eyes, or liver can also be affected. The exact cause is unclear.

How is it treated?

The goal is to allow a flare-up to subside and then keep the disease in remission for as long as possible. Which treatments are suitable depends on the type, extent, and activity of the inflammation.

  • Aminosalicylates such as mesalazine: especially for mild to moderate ulcerative colitis, as a tablet, suppository, foam or enema
  • Cortisone: for a limited time during more severe episodes
  • Immunosuppressants and biologics: in case of frequent flare-ups or severe course

According to current guidelines, the benefit of mesalazine for Crohn's disease is not sufficiently proven, even for preventing relapses. Therefore, treatment is always determined by a specialist.

Side effects and monitoring

Common side effects of mesalazine include diarrhea, abdominal pain, nausea, headache, and skin rash. Rarely, the heart or pancreas are affected; very rarely, blood disorders, kidney, liver, or lung problems occur. Severe skin reactions and kidney stones have also been reported.

Blood counts, kidney and liver function tests, and urine analysis are therefore performed before and regularly during treatment. Mesalazine is not suitable for patients with an allergy to mesalazine or salicylates, or in cases of severe liver or kidney dysfunction. According to the German prescribing information, it is not recommended for patients with impaired kidney function or an increased tendency to bleed.

Mesalazine preparations release the active ingredient differently and should not be changed without consulting a doctor.

Warning signs: When immediate help is needed

A severe flare-up or complication can become life-threatening. Call emergency services immediately on 112 (Switzerland: 144) if:

  • very severe abdominal pain occurs, or the abdomen is hard and bloated.
  • a lot of blood is passed with the stool, or dizziness and circulatory weakness occur.
  • Vomiting begins and neither stool nor gas is passed.

If you experience more than six bloody bowel movements per day, fever, or a rapid pulse, you should have a personal examination on the same day. This also applies to patients taking mesalazine if they have fever with a rash or blisters, severe headaches with abdominal cramps or visual disturbances, unexplained bleeding or bruising, persistent sore throat, chest pain, new shortness of breath, or yellowing of the skin.

Living with IBD

Even after symptoms subside, chronic inflammatory bowel disease remains an ongoing challenge. In ulcerative colitis, regularly administered maintenance therapy is associated with a lower risk of relapse. Several habits can make daily life easier:

  • Take the tablets at fixed times and do not stop taking them on your own, even if you feel well.
  • Note down follow-up appointments and lab results and take them with you to every doctor's visit.
  • Drink plenty of fluids throughout the day, especially if you have diarrhea.

There is no single diet that suits everyone affected by Crohn's disease. Nutritional counseling can be helpful in cases of weight loss or deficiencies such as iron deficiency. Regular colonoscopies are recommended after a prolonged illness because the risk of colon cancer may be increased.
