What you should know about irritable bowel syndrome and diarrhea

Acute diarrhea usually clears up on its own after a few days, whereas irritable bowel syndrome often lasts a long time. These medications alleviate symptoms, but they do not replace a medical evaluation of new, persistent, or unusual symptoms.

What is irritable bowel syndrome?

In irritable bowel syndrome (IBS), the intestines are more sensitive and function differently than usual, without any identifiable organ disease. Typical symptoms include:

  • Recurring abdominal pain or cramps that change with bowel movements
  • Diarrhea, constipation, or alternating between the two.
  • Bloating and the feeling of not completely emptying the bowels

A doctor makes the diagnosis if the symptoms persist for at least three months and other causes have been ruled out. Stress and certain foods can worsen the symptoms. Acute diarrhea, on the other hand, is usually caused by an infection and often clears up on its own after a few days.

How do the preparations work?

All preparations in this area alleviate symptoms, but do not treat their cause.

  • Butylscopolaminium bromide (Buscopan): Relaxes the muscles of the stomach and intestines, for short-term use as needed.
  • Mebeverine: Relieves cramps directly in the intestinal muscle, for regular use in cases of irritable bowel syndrome.
  • Loperamide: slows down bowel movements in cases of acute or diagnosed chronic diarrhea

When suffering from diarrhea, replacing fluids and electrolytes is paramount, for example with an electrolyte solution from the pharmacy. Which active ingredient is suitable depends on the main symptoms, pre-existing conditions, and other medications being taken.

Side effects and cautions

Buscopan can occasionally cause dry mouth, rapid heartbeat, or skin reactions. It is not suitable for patients with, among other conditions, narrow-angle glaucoma, myasthenia gravis (a pathological muscle weakness), urinary retention, bowel obstruction, or a persistently rapid heartbeat. With loperamide, constipation, flatulence, nausea, and dizziness are possible. It is not suitable for patients with fever, blood in the stool, diarrhea following antibiotic use, or an acute flare-up of ulcerative colitis. Mebeverine has few known side effects, primarily allergic skin reactions.

Taking more loperamide than the permitted daily amount can trigger dangerous cardiac arrhythmias. During pregnancy and breastfeeding, the doctor carefully considers the risks and benefits of each active ingredient.

When should you see a doctor immediately?

  • very severe or rapidly increasing abdominal pain occurs, or the abdomen is hard
  • Vomiting occurs, and neither stool nor gas is passed.

This also applies if acute diarrhea does not improve after two days, starts after taking antibiotics, or if you experience signs of dehydration such as very little urine.

Daily life with irritable bowel syndrome

Relieving the burden on the intestines in everyday life

For irritable bowel syndrome (IBS), small changes in daily routine often help. Keeping a symptom diary, noting meals, bowel movements, stress levels, and medications taken, can reveal what triggers symptoms after a few weeks. It also provides a good basis for discussion with a doctor. While not every trigger can be avoided, many sufferers find a calm, regular daily routine helpful.

  • Eat regular meals in a relaxed atmosphere, chew thoroughly, and drink enough fluids.
  • Exercise in everyday life and relaxation techniques to combat stress
  • In case of diarrhea, replace fluids and salts, for example with an electrolyte solution.
