What you should know about gout

Gout is a common inflammatory joint disease associated with elevated uric acid levels. Medications can relieve attacks and lower uric acid, but they do not replace a medical examination.

What is gout?

Gout is a metabolic disorder characterized by excess uric acid in the blood. The uric acid can precipitate as fine crystals in the joints, causing severe inflammation. Typical symptoms of a gout attack include:

  • Sudden, very severe pain, often at night and frequently at the base of the big toe.
  • a swollen, reddened and warm joint that can barely tolerate being touched
  • Symptoms subside after a few days to about two weeks

Gout is exacerbated by genetic predisposition, obesity, alcohol, a purine-rich diet, certain diuretics, and impaired kidney function. Without treatment, attacks can become more frequent, and joints and kidneys can be permanently damaged.

How do gout medications work?

The treatment pursues two goals: to alleviate the acute attack and to lower uric acid levels in the long term.

  • Colchicine: Inhibits the inflammatory response to uric acid crystals, in acute attacks and for the prevention of attacks at the beginning of uric acid lowering therapy.
  • Allopurinol: It inhibits the formation of uric acid and is taken daily as a long-term treatment, even without symptoms.

Allopurinol is also used to treat uric acid stones and kidney damage caused by uric acid. In acute attacks, anti-inflammatory painkillers or cortisone may also be considered, depending on the situation. The doctor will decide on the appropriate treatment based on symptoms, blood test results, and pre-existing conditions.

Side effects and cautions

Allopurinol: The most common side effect is a skin rash. If this occurs, the medication should be stopped immediately and a doctor should be notified, as severe skin and hypersensitivity reactions can develop in rare cases. Gout attacks may occur more frequently at the beginning of treatment.

Colchicine: Even a slightly excessive dose can cause poisoning. Nausea, vomiting, abdominal pain, or diarrhea can be the first signs; if these occur, the medication should be discontinued. Many medications increase colchicine levels, such as clarithromycin, certain antifungal and HIV medications, ciclosporin, and verapamil. Women who can become pregnant must use reliable contraception during and for at least three months after treatment. Men must not father a child during and for at least six months after treatment.

Warning signs: When immediate help is needed

Not every hot, swollen joint is gout. Call emergency services immediately on 112 (Switzerland: 144) if:

  • If severe or bloody diarrhea, violent vomiting, circulatory weakness or confusion occur after taking colchicine, or if too many tablets have been taken.
  • A skin rash with blisters, peeling skin, or sores on the mouth, eyes, or genitals develops while taking allopurinol.
  • Swelling of the face, lips or throat, or shortness of breath occurs

Fever or chills accompanied by a hot, swollen joint may indicate a joint infection. In such cases, a personal examination at a doctor's office, the on-call medical service, or an emergency room is necessary on the same day.

What helps in everyday life

Medication is often an important component of gout treatment, but lifestyle also influences uric acid levels. A strict diet is usually unnecessary; a balanced daily routine is more helpful. During uric acid-lowering treatment, the uric acid level is regularly monitored until the agreed target value is reached. Keep a record of your readings and any attacks; this will facilitate discussions with your doctor.

  • Low alcohol consumption, especially beer and spirits, and low consumption of sweetened drinks containing fructose.
  • Purine-rich foods such as offal, seafood, and large quantities of meat should only be consumed in moderation.
  • Drink enough water throughout the day and gradually lose excess weight.

Radical fasting can temporarily increase uric acid levels and trigger an attack. During an attack, many sufferers find it comforting to rest the affected joint, elevate it, and gently cool it. Do I need a prescription for allopurinol or colchicine?

Yes. Both medications require a prescription. A licensed physician will review your questionnaire to determine if a prescription is medically appropriate. Allopurinol does not relieve an acute attack. A rapid reduction in uric acid levels can dislodge crystals in the joint and initially worsen the inflammation. According to the prescribing information, treatment therefore only begins once the attack has completely subsided. Anyone already taking allopurinol should not stop taking it on their own during an attack, but should discuss the procedure with their doctor.

Allopurinol is usually used for long-term treatment of gout. With regular use, uric acid levels decrease, and deposits can dissolve over several months. Attacks sometimes occur more frequently in the first few weeks, so colchicine is often prescribed as a preventative measure. The dose is adjusted based on regular monitoring of uric acid and kidney function. If allopurinol is discontinued without consulting a doctor, uric acid levels will rise again.

Colchicine has a narrow safety margin. Never take more than prescribed, count the tablets per attack, and stop taking it if you experience nausea, vomiting, or diarrhea. Many medications and grapefruit juice increase colchicine levels. Keep the tablets out of the reach of children, as even a few tablets can be life-threatening for a child.
