Is PrEP an option for you?

What you should know about PrEP

PrEP is a component of HIV prevention and requires regular intake, testing, and medical check-ups.

What is PrEP and who is it for?

PrEP stands for pre-exposure prophylaxis: HIV-negative people take a preventative medication to reduce the risk of sexual HIV transmission. It is intended for people at increased risk of HIV, for example, those who:

  • Sex without a condom with multiple partners
  • Sex with a person with HIV whose viral load is not reliably below the detection limit
  • a recently occurring sexually transmitted infection

PrEP is intended as part of safer sex practices. Condoms are the primary protection against other sexually transmitted infections.

Tests before and during PrEP

Before starting PrEP, an HIV infection must be ruled out by a recent test. If PrEP is taken with an undetected infection, the virus can become resistant to the active ingredients. The following are also required beforehand:

  • a check of kidney function
  • a test for hepatitis B
  • Usually tests for other sexually transmitted infections are also performed.

During treatment, HIV testing is repeated at least every three months, and kidney function is monitored regularly. The tests are conducted locally, for example, at a doctor's office, public health department, or testing and counseling center.

Possible side effects

Especially at the beginning, nausea, diarrhea, headaches, or dizziness may occur. Tenofovir disoproxil can also strain the kidneys and reduce bone density. Therefore, regular check-ups are important.

In cases of hepatitis B infection, liver disease can worsen significantly after discontinuation of the medication. Caution is also advised with medications that strain the kidneys, such as high doses or multiple NSAID painkillers like ibuprofen or diclofenac.

When you need immediate medical help

If you have had contact with someone who may be at risk of HIV in the last 72 hours and are not yet taking effective PrEP, you must act quickly: Seek immediate medical attention at an HIV specialist clinic or a hospital emergency room to have your eligibility for post-exposure prophylaxis (PEP) assessed. This should ideally be done within 24 hours.

Furthermore, contact a local doctor promptly if you experience flu-like symptoms such as fever, rash or swollen lymph nodes while taking PrEP, or if you develop new bone pain and muscle weakness. Call emergency services 112 (Switzerland: 144) if you experience shortness of breath or swelling of the face.

PrEP is a component of preventive care

PrEP can only reduce the risk of HIV infection if the tablets are taken regularly. It does not protect against syphilis, gonorrhea, chlamydia, herpes, or hepatitis. Regular testing and, depending on the situation, condoms remain essential preventative measures. Open communication with partners makes it easier to find suitable protective measures together.

  • Link taking the medication to a fixed routine, such as brushing your teeth, or use a reminder on your smartphone.
  • Schedule your check-up appointments every 3 months well in advance and keep your results safe.
  • Do not stop PrEP on your own: How long to continue taking it after the last risky exposure should be determined by a doctor. This is especially important in the case of hepatitis B.

Good to know: According to current knowledge, HIV is not sexually transmissible if a person with HIV's viral load is permanently below the detection limit due to treatment. When does PrEP start working?

The active ingredients first need to accumulate in the tissue. How long this takes depends, among other things, on the type of contact; generally, several days of regular intake are necessary.
