Which treatment might be suitable?

  2. 2 If you would prefer to take a capsule Doxycycline 40 mg (Efracea) is taken once daily in the morning on an empty stomach, 2 to 3 hours apart from dairy products and minerals. It can make the skin more sensitive to light. Your doctor will decide whether this capsule is suitable for you.
  3. 3 If you are pregnant, want to become pregnant, or are breastfeeding Efracea should not be taken during pregnancy, and the cream is also not recommended during pregnancy. Careful consideration is necessary during breastfeeding. Please indicate this on the questionnaire. The doctor will decide.
  4. 4 If your eyes are also affected or you are taking isotretinoin Efracea is not intended for use if the eyes are involved, and the cream does not treat the eyes; therefore, an ophthalmological examination is advisable. Efracea should not be taken with isotretinoin. The doctor will decide.

What you should know about rosacea

Rosacea is a common, chronic skin condition affecting the face. Treatment can reduce inflammatory pimples and pustules, but the tendency for flare-ups often persists.

What is rosacea?

Rosacea is a chronic inflammatory skin condition of the face that usually begins in adulthood and often occurs in flare-ups. It primarily affects the cheeks, nose, chin, and forehead. Typical symptoms include:

  • Persistent redness and sudden flushing, often with visible blood vessels
  • Inflammatory nodules (papules) and pustules, unlike acne without blackheads.
  • Burning, stinging, or sensitive, dry skin

Some sufferers also experience eye irritation or inflammation of the eyelids. The exact cause is not fully understood. Flare-ups can be triggered by factors such as sun exposure, heat, alcohol, spicy foods, or stress.

How effective are the treatments?

Both products in this category are intended for the inflammatory pimples and pustules of rosacea (papulopustular rosacea). They are not intended for redness without pimples or for visible blood vessels.

  • Ivermectin (Soolantra): Topical cream with anti-inflammatory properties that also kills hair follicle mites (Demodex).
  • Doxycycline 40 mg (Efracea): Capsule in such a low dose that it primarily reduces inflammation and does not inhibit bacteria.

Improvement is usually only noticeable after a few weeks. Symptoms may return after discontinuing the medication. The doctor will decide which treatment is appropriate and how long it will last.

Side effects and cautions

The cream may cause burning, itching, or dryness of the skin. Initially, rosacea may temporarily worsen, but this usually improves within a week. If severe skin reactions occur, discontinue use.

Side effects of doxycycline include inflammation of the nose and throat, headaches, diarrhea, and upper abdominal pain. The skin becomes more sensitive to light, so strong sunlight and tanning beds should be avoided. Efracea should not be taken during pregnancy, in cases of allergy to tetracyclines, or in combination with oral retinoids such as isotretinoin.

Efracea is not intended to treat infections and is not interchangeable with doxycycline for the treatment of infections.

When should you see a doctor immediately?

Not every redness on the face is rosacea. Call emergency services immediately on 112 (Switzerland: 144) if:

  • Sudden swelling of the face, lips, tongue, or throat
  • Shortness of breath, hives, or rapid heartbeat with weakness may occur.

Seek immediate medical advice if you experience severe headaches accompanied by visual disturbances, severe or bloody diarrhea, yellowing of the skin or eyes, or a severe skin reaction with blisters. Painful, light-sensitive eyes or impaired vision should be examined by an ophthalmologist on the same day. You should also seek prompt medical attention if you have an uncertain diagnosis, a thickening of the nose, or pimples that do not improve after three months.

Avoid flare-ups

Protect your skin, avoid triggers

Besides treatment, daily life plays a significant role in managing rosacea. Many sufferers notice that certain triggers cause flare-ups with redness or new breakouts, such as sun, heat, hot drinks, alcohol, spicy foods, or stress. These triggers vary from person to person. Keeping a short diary with photos taken in daylight can help identify patterns and assess the progress of treatment.

  • Use a sunscreen with a high sun protection factor daily; this is especially important when taking doxycycline.
  • Cleanse your face with lukewarm water and mild, fragrance-free products, without rubbing or exfoliating.
  • Apply cosmetics and make-up only after a cream for rosacea has been absorbed and dried.

Do I need a prescription for the treatment?

Yes. Soolantra and Efracea are prescription medications for adults with inflammatory pimples and pustules associated with rosacea. A licensed physician will review your questionnaire to determine if a prescription is medically appropriate. A photograph of your face in daylight and without makeup will aid in the assessment. Please provide complete information about any pre-existing medical conditions and medications.

Rosacea usually doesn't improve within a few days. Studies have shown that the cream shows an effect after about four weeks; if there is no improvement after three months, treatment is discontinued. With the capsules, the doctor assesses the effect after six weeks. At the beginning of treatment with the cream, symptoms may temporarily worsen.

Both preparations are intended for inflammatory pimples and pustules. Persistent redness or visible blood vessels without pimples are not within their scope of application. If redness is your primary concern, please indicate this in the questionnaire. For dilated blood vessels, there are other treatments available, which can be discussed with a dermatologist.

The capsule is taken once daily in the morning on an empty stomach with a full glass of water, either sitting or standing. A 2- to 3-hour interval should be observed between taking the capsule and dairy products, heartburn medication, or supplements containing calcium, magnesium, iron, or zinc. The skin will become more sensitive to light, therefore sun protection and avoiding tanning beds are important. Discontinue use if sunburn occurs.

In that case, no prescription will be issued and you will receive a full refund of all payments made. If the diagnosis is uncertain or if the eyes are affected, we recommend consulting a local dermatologist or ophthalmologist. In case of warning signs such as swelling of the face, lips, or throat accompanied by difficulty breathing, call emergency services immediately.
