What you should know about reflux

Occasional heartburn is common and usually harmless. However, if you experience regular symptoms or warning signs, you should consult a doctor before starting any prolonged treatment.

What is reflux?

In reflux, acidic stomach contents flow back into the esophagus. Normally, a sphincter muscle at the entrance to the stomach prevents this. Typical symptoms include:

  • Burning sensation behind the breastbone, often after eating or when lying down.
  • sour burps or a sour taste in the mouth
  • A tickly cough or hoarseness, especially at night

If the symptoms occur regularly, doctors diagnose gastroesophageal reflux disease (GERD). The acid can inflame the esophagus (reflux esophagitis). Factors contributing to reflux include being overweight, eating large or late meals, alcohol, smoking, and pregnancy.

How do proton pump inhibitors work?

Proton pump inhibitors block the proton pump in the stomach lining that releases acid into the stomach. This reduces the production of stomach acid, allowing irritated mucous membranes to heal.

  • Esomeprazole: 20 or 40 mg, with or without food
  • Pantoprazole: 20 or 40 mg, about 1 hour before a meal

According to the product information, both active ingredients are used to treat reflux disease, inflammation of the esophagus, as stomach protection during long-term NSAID use, and in combination with antibiotics for Helicobacter pylori infection. Depending on the active ingredient and strength, they may also be used to heal stomach or duodenal ulcers. The full effect is achieved after a few days of regular use.

Side effects and cautions

Possible side effects include headaches, dizziness, abdominal pain, bloating, nausea, diarrhea, or constipation. Benign polyps often form in the stomach lining. If taken for several months, magnesium and vitamin B12 levels in the blood may decrease; if taken for more than a year, the risk of bone fractures increases slightly.

Both active ingredients are unsuitable for patients with an allergy to proton pump inhibitors. Interactions exist with, among others, HIV medications, blood thinners, methotrexate, and some antifungal medications.

If you experience a skin rash with blisters, fever or joint pain, stop taking the medication and seek medical advice immediately.

When should you see a doctor immediately?

Not every burning sensation in the chest originates in the stomach. Call emergency services immediately on 112 (Switzerland: 144) if:

  • Pressure or pain in the chest occurs, accompanied by shortness of breath, sweating, or radiation to the arm, neck, or jaw.
  • If blood or coffee-ground-like material is vomited, or if the stool is black

You should consult a doctor promptly if you experience difficulty swallowing, repeated vomiting, unintentional weight loss, or unexplained anemia. This also applies if you experience new or changed stomach problems after the age of 55, or if your symptoms do not improve after about four weeks of treatment.

Dealing with reflux in everyday life

Besides medication, everyday life can also influence reflux symptoms. Not every trigger affects everyone the same way. A short symptom diary can show whether certain foods, drinks, or times of day play a role. Note when the burning sensation occurs, what you ate beforehand, and whether you lay down afterward.

  • Smaller meals and the last meal about 2 to 3 hours before bedtime.
  • Slightly elevate the head of the bed if the symptoms occur mainly at night.
  • If you are overweight, reduce your weight, drink less alcohol and quit smoking.

The use of proton pump inhibitors is regularly monitored by a doctor to ensure the lowest effective dose is used. Do I need a prescription for esomeprazole or pantoprazole?

Acid-suppressing effects begin immediately after ingestion, but reach their full strength only after several days of regular use. With reflux symptoms, improvement usually occurs within 2 to 4 weeks. If there is no improvement or if the symptoms change, consult your doctor so that the cause can be investigated.

The duration of treatment is determined by the doctor. According to the product information, treatment for esophagitis usually lasts 4 to 8 weeks, but may be longer to prevent relapses or to protect the stomach. Taking the medication for several months can lead to a decrease in magnesium and vitamin B12 levels, and the risk of bone fractures increases slightly if taken for more than a year. Therefore, the treatment is regularly reviewed.

It depends on the medication. Both active ingredients must not be combined with oral rilpivirine, and esomeprazole should also not be combined with nelfinavir. Atazanavir is not recommended with either, and clopidogrel should not be combined with esomeprazole. For blood thinners, methotrexate, and some antifungal medications, the doctor will review the combination. Therefore, please list all medications you are taking on the questionnaire.
