What you should know about malaria prevention

Good preparation combines mosquito protection, appropriate medication, and knowledge of warning signs after the trip.

What is malaria?

Malaria is an infectious disease caused by parasites (Plasmodium) and transmitted through the bite of the Anopheles mosquito. The disease is prevalent in parts of Africa, Asia, and Central and South America.

Symptoms appear at the earliest about a week after the bite, sometimes only months later:

  • Fever, chills and sweats
  • Headaches and body aches, fatigue
  • Nausea, vomiting or diarrhea

Malaria tropica, in particular, can quickly become life-threatening.

Travel medicine consultation

Whether preventative medication (chemoprophylaxis) is advisable depends on the destination, time of year, duration of the trip, and type of travel. For some regions, emergency medication for use while traveling is recommended instead of continuous medication, while for others, consistent mosquito protection is sufficient.

The recommendations are updated regularly. Therefore, contact a tropical or travel medicine clinic well in advance, ideally several weeks before departure. They can also provide information on vaccinations. Please provide your exact travel itinerary in the questionnaire.

Possible side effects

Headaches, nausea, abdominal pain, vomiting or diarrhea are common side effects of atovaquone/proguanil.

Mefloquine can cause gastrointestinal problems, as well as sleep disturbances, nightmares, dizziness, anxiety, depressive moods, and, rarely, psychosis. These side effects can persist long after discontinuation of the medication.

If you experience restlessness, anxiety, confusion or mood changes while taking mefloquine, stop taking it and contact a doctor immediately. Another form of prevention is then necessary.

Chloroquine can cause gastrointestinal problems, headaches, visual disturbances, or skin rashes, and can affect heart rhythm and blood sugar levels. If you experience rapid heartbeat, fainting, or visual disturbances, seek immediate medical attention.

Fever during or after travel

No prophylaxis offers complete protection. Any fever during or after a stay in a malaria area should be investigated by a doctor immediately, even weeks or months after returning. Be sure to mention your travel destination so that you can be specifically tested for malaria.

Call emergency services 112 (Switzerland: 144) if confusion, seizures, severe shortness of breath, yellowing of the skin or disturbances of consciousness occur. Online consultation does not replace a personal examination in the case of fever after a trip to the tropics.

Avoiding stings remains the basic principle.

Tablets for prevention reduce the risk of infection, but do not offer complete protection. Anopheles mosquitoes bite primarily at dusk and at night. Other mosquitoes, such as those that transmit dengue fever, are also active during the day. Therefore, consistent mosquito protection is essential for any trip to the tropics.

  • Apply mosquito repellent containing active ingredients such as DEET or Icaridin to exposed skin and reapply regularly.
  • Wear light-colored, long and preferably tightly woven clothing, which can also be treated with insect repellent.
  • Sleep under an impregnated mosquito net or use mosquito-protected, air-conditioned rooms.

Continue taking the tablets as prescribed even after your return, as this phase is part of the preventative treatment. Plan to order them well in advance of your departure. A thermometer in your travel first-aid kit will help you detect fever early while traveling, and it's helpful to know where the nearest medical care is located. Do I even need prophylaxis for my trip?

This depends on the malaria risk at your destination, as well as the time of year, duration, and type of trip. For some regions, preventative medication is recommended, while for others, mosquito repellent or emergency medication is advised. Consult a travel medicine specialist as early as possible and provide your complete itinerary in the questionnaire. The doctor will review your request.

Malaria parasites transmitted shortly before leaving a malaria-endemic area can continue to develop in the body for some time. Taking the medication after returning home is intended to cover these stages as well. For atovaquone/proguanil, this is 7 days; for mefloquine and chloroquine, it is 4 weeks after leaving the malaria-endemic area. Therefore, do not stop taking the medication prematurely.

All medications can interact with other drugs. For example, atovaquone/proguanil interacts with rifampicin, certain HIV medications, metoclopramide, and anticoagulants such as warfarin. Mefloquine and chloroquine interact with, among other things, medications that affect heart rhythm and anti-epileptic drugs; chloroquine also interacts with amiodarone and antacids. Therefore, please list all medications you are taking on the questionnaire.

Special recommendations apply to pregnant and breastfeeding women. Travel to a malaria area should be carefully considered during pregnancy, as malaria can be particularly severe in this age group.
