What you should know about migraines

Migraine is a common neurological disorder. Triptans can relieve acute attacks, but they do not replace a medical evaluation of new or changing headaches.

What is migraine?

Migraine is a neurological disorder characterized by recurring headache attacks. Typical symptoms include:

  • Throbbing, often one-sided headaches that can last 4 to 72 hours if left untreated.
  • Nausea or vomiting, as well as sensitivity to light and sound
  • Symptoms worsen with physical activity

Some sufferers experience an aura beforehand, such as visual disturbances like flashing lights or tingling sensations. Possible triggers include lack of sleep, stress, skipped meals, or hormonal fluctuations. Women are affected about two to three times more often than men; most sufferers are symptom-free between attacks.

How do triptans work?

Triptans are medications specifically for the acute treatment of migraine attacks. These medications work via serotonin receptors, constricting dilated blood vessels in the head and inhibiting pain-transmitting neurotransmitters.

  • Rizatriptan (Maxalt): Tablet
  • Sumatriptan (Imigran): Tablet, nasal spray or injection under the skin

Triptans are only used during an attack, not preventively. They are not intended for other types of headaches, such as tension headaches. How well a triptan works and how well it is tolerated varies from person to person. For milder attacks, over-the-counter painkillers may also be an option.

Side effects and cautions

Common side effects include dizziness, fatigue, tingling, a feeling of heat, and a sensation of heaviness or tightness in the chest, throat, or neck. These symptoms usually subside quickly. However, severe or persistent chest pain after use must be evaluated by a doctor immediately.

Triptans are not suitable for patients with coronary artery disease, after a heart attack or stroke, in cases of circulatory disorders, and with uncontrolled high blood pressure. Caution is advised when used with certain antidepressants (serotonin syndrome).

If headache medication is used regularly on ten or more days a month, the headaches themselves can worsen.

When should you see a doctor immediately?

Not every headache is a migraine. Call emergency services immediately on 112 (Switzerland: 144) if:

  • suddenly the worst headache of your life occurs
  • New symptoms such as paralysis, speech disorders, or confusion may occur.
  • Headaches accompanied by fever and neck stiffness, or occurring after a head injury

Seek prompt medical attention if you experience headaches for the first time after the age of 50, if your known headache pattern changes, if attacks become more frequent despite treatment, or if an aura lasts for more than one hour.

Preventing seizures

Know your own rhythm

Many people with migraines benefit from knowing their personal triggers. A headache diary helps to identify patterns, such as connections to sleep, stress, meals, or the menstrual cycle. Note the onset, duration, and intensity of headaches, accompanying symptoms, and any medications taken. This also provides a good basis for discussion with your doctor. While not every trigger can be avoided, many sufferers find a consistent daily routine helpful.

  • Regular sleep-wake cycle, regular meals and drinking enough fluids
  • Regular endurance exercise and relaxation techniques such as progressive muscle relaxation
  • Use acute medications on no more than ten days per month.

If you experience frequent or particularly distressing seizures, preventative treatment (prophylaxis) may be advisable. You should discuss this with a local doctor, such as a neurologist. Do I need a prescription for triptans?

Yes. All triptans in this category require a prescription. They are only prescribed when a diagnosis of migraine has been clearly established. A licensed physician will review your questionnaire to determine whether a prescription is medically appropriate. Therefore, please provide complete information about any pre-existing conditions and medications you are currently taking.

Triptans are used during a migraine attack, ideally early in the headache phase, and not preventively. If the first dose is ineffective, a second dose should not be taken during the same attack. If the headache returns after improvement, the intervals and maximum doses specified in the package leaflet apply.

All forms contain a triptan, but they differ in their method of administration: tablets via the gastrointestinal tract, nasal spray via the nasal mucosa, and injection directly under the skin. Nasal spray and injection can be an option if nausea or vomiting makes oral administration difficult. The doctor will decide which form is suitable.

It depends on the medication. Triptans should not be combined with, among other things, MAOIs, ergotamines, or another triptan. Caution is advised with certain antidepressants, and a dose adjustment is necessary when taking Maxalt with propranolol. Therefore, please list all medications in the questionnaire.
