What you should know about osteoporosis

Osteoporosis often develops over years without symptoms. Medication can reduce the risk of fractures, but it does not replace exercise, fall prevention, and an adequate intake of calcium and vitamin D.

What is osteoporosis?

In osteoporosis (bone loss), bone mass and bone quality decrease. The bones become brittle and can break even under slight stress, such as from a fall while standing.

  • The vertebrae, hip, and wrist are frequently affected.
  • Vertebral fractures can cause back pain, loss of height, and a rounded back.
  • For a long time, there are often no symptoms.

Risk factors include advanced age, declining estrogen levels after menopause, long-term use of cortisone, smoking, lack of exercise, and being underweight. Osteoporosis is usually diagnosed with a bone density measurement (DXA) or after a bone fracture without a major accident.

How do the medications work?

Bone tissue is constantly being broken down and rebuilt. In osteoporosis, bone breakdown predominates. Both active ingredients in this area slow down bone loss in different ways.

  • Alendronic acid: Bisphosphonate, as a tablet daily with 10 mg or once weekly with 70 mg
  • Raloxifene (Evista): Selective estrogen receptor modulator, one tablet daily, for postmenopausal women only.

Bone density changes only over several months. Therefore, treatment is designed to be long-term, and the doctor regularly checks whether it is still effective. Calcium and vitamin D supplements are often recommended if dietary intake is insufficient.

Side effects and cautions

Alendronic acid: Common side effects include abdominal pain, indigestion, and bone, muscle, or joint pain. If the tablet is not swallowed according to the instructions, the esophagus can be damaged. Rarely, damage to the jawbone or unusual fractures of the femur occur. Therefore, good oral hygiene and regular dental checkups are important.

Raloxifene: Common side effects include hot flashes, leg cramps, and swelling of the hands or legs. Raloxifene increases the risk of blood clots in the legs, lungs, or eyes and is discontinued before prolonged bed rest. After a stroke or in cases of high stroke risk, the doctor will monitor the patient particularly carefully.

When should you see a doctor immediately?

Call emergency services immediately on 112 (Switzerland: 144) if:

  • sudden shortness of breath, chest pain or coughing of blood
  • one side of the body suddenly becomes weak or numb, or speech disorders occur
  • Swelling of the face, lips or throat, or vomiting of blood

Seek immediate medical attention if you experience pain when swallowing or behind the breastbone, black stools, a painfully swollen leg, sudden vision problems, or vaginal bleeding. Also report any discomfort in the mouth or jaw, as well as any new pain in the thigh, hip, or groin. A bone fracture should be medically evaluated after a fall that causes severe pain.

What you can do yourself

Medication is only one part of osteoporosis treatment. Exercise, nutrition, and the living environment play a crucial role in maintaining strong bones and reducing falls. Many of these measures can be easily integrated into daily life.

  • Regular exercise with strength and balance exercises, for example walking, gymnastics or dancing.
  • A calcium-rich diet including dairy products, green vegetables and nuts, as well as time spent outdoors for vitamin D.
  • Remove tripping hazards such as loose carpets and cables, ensure good lighting, and wear sturdy footwear.

Quitting smoking and limiting alcohol consumption also protect bones. Calcium or vitamin D supplements should only be taken on the advice of a doctor; alendronic acid should be taken at least 30 minutes after taking the tablet. Do I need a prescription for osteoporosis tablets?

Yes. Alendronic acid and Evista are prescription medications. Based on your questionnaire, a licensed physician will assess whether you have osteoporosis or an increased risk of fractures and whether a prescription is medically appropriate. A bone density measurement report, which you can upload, will be helpful. Please provide complete information about any pre-existing conditions and medications.

Swallow the tablet whole in the morning on an empty stomach, immediately after waking up, with a large glass of tap water. Remain upright for at least 30 minutes afterward and wait until then before consuming breakfast, drinks, or other medications. Coffee, tea, milk, juice, and mineral water are not suitable for taking the tablet. These instructions protect the esophagus and ensure that the active ingredient is absorbed.

Evista is intended only for postmenopausal women who are no longer able to become pregnant. It is used to treat and prevent osteoporosis. It is not suitable for use in cases of, among other things, thrombosis, liver disease, severe kidney disease, or unexplained vaginal bleeding. Evista does not help with hot flashes; in fact, it can worsen them.

Osteoporosis is usually treated over several years because bone density changes slowly. The doctor regularly checks whether the treatment is still appropriate. With alendronic acid, this is reassessed particularly after about five years. Do not stop taking it on your own; discuss any changes with your doctor beforehand.

In that case, no prescription will be issued, and you will receive a full refund of all payments made. If osteoporosis has not yet been diagnosed, or if you are experiencing problems with your esophagus, jaw, or legs, we recommend scheduling an appointment with your general practitioner or gynecologist.
