What you should know about vaginosis and trichomoniasis

Bacterial vaginosis is one of the most common causes of altered vaginal discharge, and trichomoniasis is one of the most common sexually transmitted infections. Both can be treated once the cause has been definitively identified, but bacterial vaginosis often recurs.

Bacterial vaginosis: Causes and symptoms

In bacterial vaginosis, the balance of the vaginal flora is disrupted. The protective lactic acid bacteria (lactobacilli) decrease, while other bacteria, such as Gardnerella vaginalis, multiply rapidly. This is promoted, for example, by new or changing sexual partners, vaginal douching, or smoking.

Typical signs are:

  • thin, greyish-white discharge
  • fishy odor, often stronger after sex or during menstruation
  • usually little itching, burning or redness

Many affected women experience no symptoms at all. Bacterial vaginosis is not considered a classic sexually transmitted infection, but it is spread through sexual contact.

Trichomonads: Transmission and signs

Trichomonas vaginalis are single-celled organisms transmitted through sexual contact. The infection (trichomoniasis) is one of the most common sexually transmitted infections worldwide. In many women and most men, it is asymptomatic.

Possible signs in women include:

  • yellowish-greenish, sometimes frothy and unpleasant-smelling discharge
  • Itching, burning and redness in the genital area
  • Pain during urination or sex

Men occasionally notice burning during urination or discharge from the urethra. Without treatment, the infection can persist for a long time and be passed on unnoticed.

Treatment and side effects

Both infections are often treated with metronidazole, an antibiotic effective against bacteria and single-celled organisms that live without oxygen. Depending on the diagnosis, the doctor will prescribe a single dose or a course of treatment lasting several days. For bacterial vaginosis, preparations applied vaginally are also an option.

Possible side effects include a metallic taste, nausea, stomach upset, diarrhea, and dark urine. Occasionally, usually with high doses or prolonged use, tingling or numbness in the hands and feet, or seizures, may occur. If you experience tingling, numbness, confusion, yellowing of the skin, or severe, persistent diarrhea, stop taking the medication and contact your doctor immediately.

No alcohol should be consumed during the treatment and for at least 48 hours afterwards.

When should I see a doctor in person or immediately?

Similar discharge can also be caused by vaginal yeast infections, chlamydia, or gonorrhea. Get yourself examined promptly if you experience these symptoms.

  • Blisters, ulcers or warts are visible in the genital area

This also applies if the symptoms do not subside after treatment or quickly recur. Tips for treatment and prevention

Bacterial vaginosis often recurs in many women after treatment, frequently within a few months. With trichomoniasis, whether reinfection occurs depends primarily on all sexual partners being treated. Certain habits can help:

  • Take the tablets for as long as prescribed by the doctor, even if the symptoms subside earlier, and be aware of hidden alcohol, for example in cough syrups or mouthwashes.
  • If you have trichomoniasis, abstain from sex until the treatment is completed for you and your sexual partners and the symptoms have subsided; after a single dose, abstain for at least one week; condoms reduce the risk of reinfection.
  • Clean the intimate area only with water or a mild, fragrance-free wash lotion and avoid vaginal douches and intimate sprays.

How can I recognize bacterial vaginosis?

A thin, grayish-white discharge with a fishy odor is typical, often worsening after sex. Itching and redness are usually absent, unlike with a yeast infection. Yellowish-green discharge accompanied by burning is more indicative of trichomoniasis. The causes can only be definitively distinguished through an examination, such as a swab and vaginal pH measurement.

Yes, in the case of trichomoniasis. Current sexual partners should be tested and treated, even if they have no symptoms. Sexual activity should be avoided until everyone has been treated and their symptoms have subsided. For bacterial vaginosis, treating male partners is not yet generally recommended. However, recent studies suggest it may reduce recurrences. This approach can be discussed with your gynecologist.

Metronidazole can cause adverse reactions when taken with alcohol, including skin reddening, nausea, vomiting, headache, or rapid heartbeat. Therefore, no alcohol should be consumed from the first tablet until at least 48 hours after the last. This also applies to hidden alcohol, such as that found in some cough syrups, drops, or mouthwashes.

Relapses are common because the protective lactobacilli don't always recover sufficiently after treatment. New sexual partners, vaginal douching, or smoking can also play a role. Metronidazole treatments should be repeated as infrequently as possible. If symptoms occur several times a year, please have the causes investigated by your gynecologist and discuss further steps.
