What you should know about the thyroid gland

Hypothyroidism is usually treated long-term and requires regular blood tests. Levothyroxine replaces the missing hormone, but a medical evaluation of new symptoms is not necessary.

What is hypothyroidism?

The thyroid gland produces the hormones thyroxine (T4) and triiodothyronine (T3), which help regulate metabolism, heartbeat, and body temperature. In an underactive thyroid (hypothyroidism), these hormones are deficient. Possible symptoms include:

  • Fatigue, lack of motivation and sensitivity to cold
  • Weight gain, constipation and dry skin
  • low mood or menstrual irregularities

Common causes include chronic inflammation of the thyroid gland (Hashimoto's thyroiditis), as well as surgery or radioiodine therapy. Because the symptoms can also have other causes, the diagnosis is always made via blood tests, especially the TSH level and free T4 (fT4).

Treatment with levothyroxine

Levothyroxine is a synthetically produced thyroid hormone and is identical to the body's own thyroxine. In the body, it is converted into the active hormone T3. According to the product information, it is used:

  • to replace the missing hormone in hypothyroidism
  • For benign enlargement of the thyroid gland (goiter) with normal function and for prevention after its surgery
  • for follow-up treatment in thyroid cancer, coordinated with the specialist practice

The dose is adjusted gradually based on the TSH level, and a change only takes full effect after several weeks. Treatment usually lasts a long time, often lifelong. Levothyroxine is not suitable for weight loss.

Side effects and cautions

Levothyroxine is generally well tolerated when taken at the appropriate dose. Side effects primarily occur when the dose is too high: palpitations, restlessness, sleep disturbances, tremors, sweating, diarrhea, or weight loss. A consistently high dose can contribute to heart rhythm disorders such as atrial fibrillation and osteoporosis.

Levothyroxine should not be taken in cases of untreated hyperthyroidism, untreated adrenal insufficiency or hypopituitarism, after a recent heart attack, or in cases of acute myocarditis. Calcium, iron, and antacids reduce its absorption. The effects of blood thinners and diabetes medications may be altered.

When should you see a doctor immediately?

Call emergency services immediately on 112 (Switzerland: 144) if:

  • Pain or tightness in the chest occurs
  • Swelling of the face, lips or throat, or shortness of breath occurs
  • a seizure occurs or fever sets in with severe restlessness, confusion and rapid heartbeat

Seek immediate medical advice if you experience a very rapid or irregular heartbeat or severe headaches accompanied by visual disturbances. You should also discuss palpitations, tremors, excessive sweating, or unintentional weight loss promptly, as the dose may be too high. This also applies if symptoms of hypothyroidism recur or if you become pregnant.

Levothyroxine in everyday life

Plan your daily intake carefully.

Levothyroxine works most consistently when taken according to the same routine every day. Even small differences in the time of day, breakfast, or the specific product can alter TSH levels because the range between too little and too much hormone is very narrow. A consistent morning routine helps, for example, keeping the pack next to your toothbrush.

  • Take the tablet in the morning on an empty stomach with water and wait at least 30 minutes before breakfast.
  • Take calcium, iron, magnesium, and heartburn medication at least 2 hours later.
  • Beware of high-dose biotin before blood tests, as it can distort lab results.

A missed dose is not taken twice; simply continue with the usual dose the following morning. Soy products can reduce absorption, so avoid sudden and drastic changes to your diet.
