What you should know about hormonal contraception

Hormonal contraception can reliably protect against pregnancy if used correctly and if there are no health reasons against it.

Pill, ring, patch or IUD?

Combination pills, vaginal rings, and contraceptive patches contain an estrogen and a progestin. These hormones primarily suppress ovulation. Depending on the specific product, a break follows, or placebo tablets are taken.

The progestin-only pill (mini-pill) does not contain estrogen and is taken continuously without a break. Hormonal IUDs release a progestin directly into the uterus and are effective for several years.

None of these methods protect against sexually transmitted infections; condoms are essential for that. Certain medications, such as St. John's wort or some anti-epileptic drugs, can reduce the effectiveness of the pill, ring, and patch; in the case of tablets, vomiting or severe diarrhea within 3 to 4 hours of taking the medication can also reduce its effectiveness.

Thrombosis risk and blood pressure

All methods containing estrogen, including combined pills, the contraceptive ring, and the patch, increase the risk of blood clots (thrombosis, pulmonary embolism), most significantly during the first year of use. The risk depends on the specific product: pills containing levonorgestrel have a lower risk, while pills containing drospirenone or desogestrel, as well as the ring and patch, have a higher risk.

The risk is further increased by:

  • Smoking, especially from age 35
  • severe obesity, advanced age, or prolonged bed rest
  • Thrombosis in parents or siblings at a young age

High blood pressure can also be a sign that estrogen is not present. Have your blood pressure checked regularly and provide complete information on the questionnaire.

Possible side effects

Especially in the first few months, spotting, breast tenderness, headaches, or nausea can occur. Irregular, less frequent, or absent bleeding is particularly common with the progestin-only pill and hormonal IUDs. The patch can irritate the skin at the application site. A hormonal IUD can, rarely, shift out of place, be expelled, or perforate the uterine wall during insertion.

Hormonal contraceptives can also affect mood. If you experience mood swings or depressive symptoms, even shortly after starting to use them, seek medical advice promptly.

Warning signs: When to see a doctor immediately.

Call emergency services immediately on 112 (Switzerland: 144) or go to an emergency room if the following signs occur during use:

  • Pain or swelling in one leg
  • sudden shortness of breath, chest pain, or coughing up blood
  • sudden severe headache, visual or speech disturbances, weakness or numbness on one side of the body
  • very severe lower abdominal pain, especially when menstruation is absent.

Consult a doctor in person if you suspect you are pregnant. This also applies if you experience fever, foul-smelling discharge, persistent abdominal pain while using a hormonal IUD, or if you can no longer feel the strings. Inform your healthcare team before any planned surgery that you are using hormonal contraception.

Safe contraception in everyday life

Regularity makes the difference

The pill is only effective if taken consistently. Many women therefore combine taking it with a fixed daily routine, such as brushing their teeth, or use a reminder on their smartphone.

  • Forgotten tablet: How long you can take it without losing protection varies depending on the product. The package leaflet describes the exact procedure.
  • Ring and plaster: Here too, the fixed change day is important. A reminder in the calendar prevents a change or the end of the break from being overlooked.
  • Tell every doctor and pharmacist that you are using hormonal contraception, because some medications weaken its effectiveness.

Do I need a prescription for contraception?

Yes. All products in this category require a prescription. A licensed physician will review your questionnaire to determine if a prescription is medically appropriate. If you start using the IUD on the first day of your period, protection is usually effective from the start. A hormonal IUD provides immediate protection if inserted within the first 7 days of your period. Smoking, especially when combined with a combined pill, contraceptive ring, or patch, increases the risk of blood clots, heart attack, and stroke, particularly after age 35. Therefore, estrogen-based methods are usually not suitable for women over 35 who smoke; an estrogen-free pill or a hormonal IUD may be an alternative.
