Preparations for eczema

Which medication might be suitable?

  1. 1 If your skin is severely inflamed, thickened, or flaky Topical cortisone reduces inflammation and itching and is used for short periods on limited areas. Betamethasone (Diprosone) is highly effective, clobetasol (Dermovate) very effective and is primarily considered when weaker preparations have not been sufficient. What you should know about eczema

Eczema is a common, non-contagious inflammation of the skin. Preparations containing active ingredients can alleviate flare-ups, but they do not replace daily skin care or the medical evaluation of unclear skin changes.

What are eczema and atopic dermatitis?

Eczema is a non-contagious inflammation of the skin. Atopic dermatitis (neurodermatitis) is a common form that often begins in childhood and occurs in flare-ups. Typical symptoms include:

  • Intense itching, especially in the evenings and at night
  • Reddened, dry or flaky skin, also weeping during acute flare-ups
  • In adults, common areas include elbows, backs of knees, hands, neck and face.

This is caused by a disrupted skin barrier and a hypersensitive immune response. Flare-ups can be triggered by dry air, sweating, stress, irritants, or allergens. Other types of eczema arise, for example, from contact with allergens or irritants.

How is eczema treated?

The foundation is regular, moisturizing skincare. During a flare-up, topical active ingredients are added:

  • Cortisone (betamethasone, clobetasol): Reduces inflammation and itching, temporarily and in limited areas.
  • Calcineurin inhibitors (pimecrolimus, tacrolimus): cortisone-free, especially for atopic dermatitis, also for the face and neck

The strength of the treatment depends on the severity and location of the eczema. After improvement, cortisone is usually applied less frequently or replaced with a weaker medication. For very extensive or severe atopic dermatitis, treatment should be carried out in a local dermatology practice, for example, for light therapy or systemic medications.

Side effects and cautions

Prolonged or extensive use of cortisone can thin the skin and cause stretch marks or visible blood vessels. It should not be applied to eyelids, infected skin, open wounds, acne, or rosacea. After prolonged use, it should not be stopped abruptly, as this can cause a severe flare-up of the condition.

Calcineurin inhibitors often cause burning or a sensation of warmth at the beginning of treatment. Strong sunlight, tanning beds, and UV light therapy should be avoided during treatment. They are not suitable for people with weakened immune systems.

Blurred vision during cortisone treatment should be examined by an ophthalmologist.

When should you see a doctor immediately?

Some skin changes require prompt medical attention. Call emergency services immediately on 112 (Switzerland: 144) if:

  • Swelling of the face, lips, tongue or throat, or shortness of breath occurs
  • a redness spreads rapidly and is accompanied by high fever, chills or confusion

You should have painful blisters or open sores that spread rapidly, often accompanied by fever, checked by a doctor on the same day. This could be a herpes infection of the eczema skin. Also warrant a prompt visit to the doctor are purulent or honey-yellow crusted areas, redness or blisters around the eye, and new nodules that look different from the eczema.

Skincare in everyday life

Strengthen the skin barrier

In atopic dermatitis, the skin's protective barrier is impaired. The skin loses moisture more easily and reacts more sensitively to external stimuli. Regular basic care with moisturizing creams or ointments is therefore part of the treatment, even during symptom-free periods. A skin diary can help identify triggers for you, such as stress, sweating, wool, or certain skincare products.

  • Take a short, lukewarm shower, gently pat your skin dry, and then apply lotion.
  • Wear soft, airy clothing and keep your fingernails short so that scratching injures the skin less.
  • Maintain the interval between applying the moisturizing cream and the active ingredient preparation as specified in the package insert; for example, at least 2 hours for Protopic.

Do I need a prescription for these creams and ointments?

Yes. All medications in this category require a prescription. A licensed physician will review your questionnaire to determine if treatment is medically appropriate and which medication is suitable. Strong and very strong cortisone is intended for short-term use on limited areas. The package insert for clobetasol (Dermovate) specifies a maximum of 4 weeks without medical supervision, while for betamethasone (Diprosone) the maximum is generally 3 weeks. After prolonged use, the treatment is gradually reduced. The amount and duration are determined by the doctor.

Both reduce inflammation in the skin, but in different ways. Cortisone has a broad anti-inflammatory effect and is used for short periods. Pimecrolimus (Elidel) and tacrolimus (Protopic) do not contain cortisone and are intended for atopic dermatitis, especially when cortisone is insufficiently effective, not tolerated, or unsuitable. The doctor decides which medication is appropriate.

Burning, a feeling of warmth, or itching at the treated area are common at the beginning and usually subside within the first week. Strong sun, tanning beds, and UV light therapy should be avoided during treatment. The cream or ointment should not be applied to infected skin, such as with cold sores. The skin may temporarily redden after consuming alcohol.
