What you should know about herpes

Herpes is widespread and usually harmless, but it can be bothersome. Knowing the signs allows for early intervention.

causes and triggers

Herpes is caused by the herpes simplex virus (HSV). Type 1 usually appears on the lips, type 2 usually in the genital area. However, both types can occur in either location, for example, through oral sex. After infection, the viruses retreat to nerve ganglia and can reactivate repeatedly. Possible triggers include:

  • Stress, fever, or infections
  • strong sun (UV radiation)
  • Menstruation or a weakened immune system

Typical course

An outbreak often begins with tingling, itching, or burning at the affected area. This is followed by the formation of small, painful blisters, which burst, scab over, and usually heal within one to two weeks.

The first genital infection is often more severe, for example with fever, swollen lymph nodes, and pain when urinating. Relapses are usually milder. Herpes is most contagious while blisters are present. However, the virus can also be transmitted without visible symptoms.

Treatment: start early

Antiviral medications containing acyclovir or valacyclovir inhibit viral replication. Treatment should begin as early as possible, ideally at the first sign of tingling, before blisters appear. Treatment regimens vary depending on the individual situation.

  • Cold sores: Aciclovir cream for several days or tablets, with valacyclovir as a short, one-day treatment
  • Genital herpes: Tablets taken over several days for first outbreak or relapse
  • Frequent relapses: Preventive long-term therapy with tablets is possible

Headaches and nausea are common side effects of the tablets; headaches are very common with valacyclovir. The cream may cause a brief burning or stinging sensation at the treatment site. The virus remains in the body despite treatment.

  • the eye is affected, for example with redness, pain, light sensitivity or visual disturbances
  • Blisters spread over a large area, especially in cases of atopic dermatitis.
  • if you are pregnant or if a newborn has had contact with herpes

  • Blisters appear in a linear pattern on one side of the body (suspected shingles)

Daily life with herpes

Protect others, know the triggers

Herpes cannot be permanently eliminated from the body. However, you can do several things to protect others and better understand outbreaks. Keeping a brief diary of potential triggers can help identify personal patterns. Since stress is a common trigger, it's worth paying attention to getting enough sleep and rest.

  • During an outbreak, avoid kissing, oral sex, and sex involving contact with the affected area.
  • Do not pop blisters, wash your hands after touching them, and do not share towels, glasses, or lip balm.
  • Protect your lips from UV radiation if the sun is a trigger for you.
