Which medication might be suitable?

What you should know about high blood pressure

High blood pressure is very common and often goes undetected for a long time. Regular measurements and consistent treatment can reduce the risk of heart attack, stroke, and kidney damage.

What is high blood pressure?

In high blood pressure (hypertension), the pressure in the blood vessels is persistently elevated. High blood pressure is usually diagnosed when readings of 140/90 mmHg or higher are repeatedly measured in a doctor's office. For home measurements, the threshold is 135/85 mmHg.

Common influencing factors include:

  • hereditary predisposition and increasing age
  • Obesity, lack of exercise, high salt intake and regular alcohol consumption
  • Conditions such as kidney disease, sleep apnea (nighttime breathing interruptions), or hormonal disorders

High blood pressure usually causes no symptoms. If left untreated, it can damage the heart, brain, kidneys, and eyes over many years.

What blood pressure lowering medications are available?

Blood pressure-lowering drugs act on different parts of the body. Guidelines primarily identify four classes of drugs as the basis for treatment, which are often combined:

  • ACE inhibitors or sartans: Enalapril, Ramipril, Candesartan, Telmisartan
  • Calcium antagonists: Amlodipine
  • Thiazide diuretics: Hydrochlorothiazide

ACE inhibitors and ARBs are generally not combined for high blood pressure. Beta-blockers are primarily considered when there is also a heart condition, such as coronary artery disease, heart failure, or arrhythmias. Spironolactone, furosemide, and clonidine are used in specific situations, for example, in cases of poorly controlled blood pressure or fluid retention.

Side effects and cautions

Dizziness and fatigue are common at the beginning of treatment or after a dose increase, especially upon standing. Typical side effects for certain drug classes include:

  • ACE inhibitors: dry, irritating cough
  • Calcium antagonists: swollen ankles
  • Beta blockers: slow pulse and cold hands and feet
  • Diuretics: Changes in potassium, sodium, and kidney function

ACE inhibitors, sartans, and spironolactone are not suitable during pregnancy. Beta-blockers and clonidine should not be stopped abruptly. Please list all medications in the questionnaire, including painkillers such as ibuprofen and potassium supplements.

Warning signs: When immediate help is needed

Severely elevated blood pressure accompanied by symptoms and some side effects can be an emergency. Call emergency services immediately on 112 (Switzerland: 144) if:

  • Chest pain, a feeling of tightness in the chest, or sudden shortness of breath may occur.
  • Paralysis, numbness, speech or vision problems, or severe headaches occur again.
  • Swelling of the face, tongue, or throat, for example while taking an ACE inhibitor
  • Fainting, a very slow or highly irregular heartbeat, or sudden muscle weakness may occur.

If your blood pressure readings are 180/110 mmHg or higher without any symptoms, rest and measure again after about 30 minutes. If the readings remain this high, seek medical advice the same day.

Living well with high blood pressure

Monitoring blood pressure in everyday life

Medication is only one part of blood pressure management. Changes in daily routine can further lower blood pressure and reduce the risk of cardiovascular disease. For home monitoring, an upper arm blood pressure monitor is suitable. Measurements should be taken after a few minutes of rest while sitting, ideally in the morning before taking medication and again in the evening.

  • Less salt, lots of vegetables, fruit and whole grain products, and little alcohol.
  • Regular endurance exercise such as brisk walking, cycling or swimming
  • Weight loss for overweight individuals, sufficient sleep and conscious breaks during periods of stress

Record your readings and bring them to your appointments. Blood pressure medication should be taken regularly, even without symptoms, and should not be discontinued without consulting a doctor. Can I get my blood pressure medication prescribed online?

Medications in this category require a prescription. If your high blood pressure has already been diagnosed and treated by a doctor, they will review your questionnaire to determine if a prescription is appropriate. Please have your current blood pressure readings, your current medication dosage, and any available lab results or medication list ready. Regular in-person checkups remain important.

High blood pressure usually causes no symptoms, but over the years it damages blood vessels, the heart, the brain, and the kidneys. Sustained blood pressure reduction can lower the risk of heart attack, stroke, and kidney damage. The effect only lasts as long as the medication is taken regularly. Therefore, do not stop taking any medication without consulting your doctor; abruptly stopping beta-blockers and clonidine can be dangerous.

Depending on the active ingredient, blood pressure reduction begins within hours to days. However, the full effect of many medications is only noticeable after about 2 to 4 weeks, and with telmisartan, sometimes after up to 8 weeks. Therefore, the dose is usually not changed prematurely. During this time, measure your blood pressure regularly and discuss the readings at your next appointment.

ACE inhibitors such as enalapril and ramipril, as well as sartans such as candesartan and telmisartan, are not suitable during pregnancy because they can harm the unborn child. Spironolactone is also unsuitable in this case. Other medications are only used after careful consideration. If you are planning a pregnancy or are pregnant, consult your doctor immediately and do not discontinue any medication on your own. Several of these medications are also unsuitable while breastfeeding.
