What you should know about Restless Legs

Restless legs syndrome is a neurological disorder that can disrupt sleep, especially in the evenings and at night. Dopamine agonists can alleviate the symptoms, but they do not replace a personal evaluation of possible underlying causes such as iron deficiency.

What is restless legs syndrome?

Restless legs syndrome (RLS) is characterized by a strong urge to move the legs. Typical symptoms include:

  • Unpleasant tingling, pulling or burning sensation in the legs, less frequently in the arms.
  • Symptoms at rest, especially in the evenings and at night
  • Improvement through movement, such as standing up or walking around

The result is often difficulty falling asleep and staying asleep, as well as daytime fatigue. Frequently, no cause is apparent. However, restless legs can also be caused by iron deficiency, kidney disease, pregnancy, or certain medications. Therefore, a personal medical examination is essential for diagnosis.

Treatment with dopamine agonists

Pramipexole and ropinirole mimic the effects of the neurotransmitter dopamine in the brain. According to the prescribing information, they are indicated for two conditions in adults:

  • Restless Legs Syndrome: Moderate to severe symptoms without any other identifiable cause; take only non-retarded tablets once in the evening.
  • Parkinson's disease: Tablets several times a day or extended-release tablets once a day

The dose is increased slowly at the beginning. In the case of restless legs syndrome, the doctor checks after about 3 months whether further treatment is necessary. For mild symptoms or iron deficiency, other measures are considered first.

Side effects and cautions

Nausea, dizziness, fatigue, and headaches are common, especially at the beginning. The following are particularly important:

  • Sudden falling asleep: Even without warning, do not drive a vehicle or operate machinery.
  • Impulse control disorders: For example, gambling addiction, shopping addiction, binge eating, or increased sexual desire
  • Augmentation: The symptoms begin earlier in the day, become more severe, or spread.

Family members should be informed of any possible changes in behavior. Dizziness upon standing and hallucinations are also possible. Do not change or discontinue the dosage or administration on your own, but only according to your doctor's instructions.

When immediate medical help is needed

Call emergency services immediately on 112 (Switzerland: 144) in the following cases:

  • high fever with stiff muscles, confusion or rapid heartbeat, especially after discontinuation or dose reduction
  • Swelling of the face, lips or throat, shortness of breath or severe skin rash
  • Fainting or delusions posing a danger to oneself or others

Report the following to your doctor promptly: sudden falling asleep during the day, hallucinations, unusual urges such as gambling or shopping addiction, and increased symptoms during treatment. Restless legs that occur for the first time during pregnancy or together with numbness and muscle weakness require a personal evaluation.

Everyday life with restless legs

Consciously shape the evening

Besides medication, everyday habits also play a role in restless legs syndrome. Many sufferers find it helpful to know their potential triggers and to consciously plan their evenings. A brief symptom diary, noting the time, intensity of symptoms, sleep patterns, and medication intake, also facilitates medical checkups after about three months.

  • Have your iron levels (ferritin) checked by your family doctor, as iron deficiency can worsen the symptoms.
  • Avoid alcohol and caffeine in the evening and maintain a regular sleep-wake rhythm.
  • Try light exercise, stretching exercises, or a foot bath before bedtime.

Some medications can worsen restless legs syndrome, for example, certain antidepressants, drowsy antihistamines, or metoclopramide for nausea. Do I need a prescription for pramipexole or ropinirole?

Yes. Both medications require a prescription. A medically diagnosed moderate to severe restless legs syndrome or a neurologically diagnosed Parkinson's disease is required.
