What you should know about skin infections

Bacterial skin infections are common and often remain confined to a small area. Antibiotics can help, but they do not replace a personal examination if the infection spreads or fever develops.

What are bacterial skin infections?

Staphylococci or streptococci are most commonly the cause of the infection. These bacteria enter through small injuries, scratches, or irritated skin. Common forms include:

  • Impetigo (cortisone): Blisters and weeping sores with honey-yellow crusts, easily transmissible
  • Folliculitis and furuncles: Inflamed hair follicles with pustules or a painful, purulent knot
  • Cellulite: A reddened, warm, and swollen area of ​​skin where the inflammation lies deeper.

Wounds or eczema can also become infected with bacteria. Fungal infections or herpes, against which antibiotics are ineffective, can present similarly.

How do antibiotics work?

Antibiotics inhibit the growth of bacteria or kill them. They are ineffective against viruses and fungi. There are two approaches in this area:

  • Ointment or cream: Mupirocin (Bactroban) or fusidic acid (Fucidin) acts directly at the infected site and is intended for small, superficial infections.
  • Capsules: Flucloxacillin is a penicillin and reaches deeper infections via the bloodstream.

Flucloxacillin is ineffective against MRSA (staphylococci that are resistant to many antibiotics). Topical antibiotics are only used for a short time to prevent the development of resistance. If the skin does not improve after a few days, a follow-up medical evaluation is necessary.

Side effects and cautions

Ointments and creams may cause burning, itching, or reddening of the treated area; occasionally, an allergic skin reaction occurs. Contact with the eyes should be avoided, and Bactroban is not suitable for use in the nose.

Gastrointestinal side effects such as nausea or diarrhea and skin rash are possible with flucloxacillin. Very rarely, hepatitis with jaundice occurs, even up to two months after treatment, and more frequently in people over 50. It is not suitable for those allergic to penicillins or cephalosporins. Anyone regularly taking paracetamol should discuss this with their doctor beforehand.

Do not save leftover antibiotics and do not pass them on to others.

When should you see a doctor immediately?

Not every skin infection can be treated online. Call emergency services immediately on 112 (Switzerland: 144) if:

  • a redness spreads quickly and is very painful, forms blisters or turns dark.
  • Shortness of breath, confusion, severe drowsiness or a very severe feeling of illness may occur.
  • Swelling of the face, lips or neck, or a stiff neck accompanied by severe headaches

If you experience fever or chills, a spreading warm redness, a red streak on your skin, an infected burn, or inflammation at a catheter site, seek immediate medical attention. Large, purulent lumps and infections of the eye, hand, or joint should be checked promptly on-site.

Many bacterial skin infections, especially impetigo, are spread via hands, towels, or close skin contact. Simple hygiene rules can reduce the risk of spreading the infection to others and to new infections on your own body. It is also important to complete the prescribed treatment and not to save any leftover medication for later use, as unnecessary or prolonged use of antibiotics promotes resistance.

  • Do not scratch or press on affected areas and wash your hands before and after application.
  • Use your own towel, wash it frequently, and cover the area with a plaster if necessary.
  • Clean small wounds immediately and regularly care for dry, cracked skin in cases of eczema.
